IRS FORM 1099G
INFORMATION ON BENEFITS PAID AND TAXES WITHHELD IN A CALENDAR YEAR
Unemployment compensation is subject to federal income tax and must be reported
as income on your federal income tax return even if you have repaid some or all of the benefits received. *
You are not required to submit a copy of your Form 1099G with your income tax return.
In January of each year, an IRS Form 1099-G is made available online to each claimant
who received unemployment compensation, state and/or federal, during the previous calendar year.
This form also shows the amount of federal income tax withheld from unemployment compensation.
The Department of Labor and Workforce Development will not mail the 1099-G forms,
however the information will be available online by the IRS deadline, January 31.

If you are unable to access your Form 1099-G Tax Information online,
you may mail or fax a written request for this information. Your request
should include the following: name printed, signature, address, date of birth,
social security number and telephone number to:
